Job Summary:

ASB is seeking a Participant Navigator who serves as a single point of contact and gatekeeper for referring organizations, participants, and students to ASB programs. Provide direct participant assistance with accessing internal and external resources. The incumbent must be able to perform the following essential functions:

Essential Functions:

  • Assist participants in understanding the resources available including informing eligible participants about education and enrichment programs and facilitated groups
  • Serve as a direct link between participants and external partnering organizations
  • Facilitate appointments for consults and support services within established service standards, including instructors, group facilitators, and enrichment programs
  • In collaboration with Director of Development and Learning, coordinate goal development and management to be used by instructors and group facilitators
  • Follow participants through the care continuum
  • Work with Education and Enrichment staff and others to collect data, track outcomes, and support strategic planning processes
  • Provide follow up and assessment of participant from initial intake through successful attainment of goals; review and develop plans for improvement in collaboration with Director of Learning and Development
  • Deliver quarterly report card to Director of Learning and Development, Director of Participant Engagement and Outreach, and Chief Program Officer which demonstrates outcomes and performance improvement activities
  • Adhere to established productivity benchmarks
  • Act professionally at all times as a representative of ASB, demonstrated by interacting positively with ASB Staff, program participants, volunteers, Board Members, organizational partners, and guests
  • Completion of work in a timely and high-quality manner, meeting established deadlines and sensitive to the needs of the ASB targeted population
  • Build and maintain internal and external stakeholder relationships
  • Provide excellent customer service
  • Perform special projects and other duties as assigned

Supervisory Responsibilities:

  • This position has no direct reports

  • Bachelor’s degree in Social Work or related field required, Master’s degree preferred
  • Experience with customer service concepts and methods for service improvement preferred
  • Experience working in the disability community, healthcare setting, or related field
  • Experience working with vulnerable population
  • Familiar with resources in the Philadelphia area preferred
  • Experience using a database to record and organize participant information. Salesforce experience preferred.
